POLICY ON REQUESTING FOR TRANSCRIPT OF RECORDS AND OTHER DOCUMENTS

(as approved on September 13, 2013. you may download an original copy of this policy here)

Fill out a request form for either a paper or an electronic document two (2) business days in advance. Return the form to the Registrar’s Office via fax, mail, in person, or email as a signed/ scanned PDF. You may get a form from the Office of the Registrar or download it from here, or request an electronic copy via email.

REQUEST IN PERSON

    • The Office of the Registrar is open on weekdays from 7:30 a.m. until 4:30 p.m., and is located at Binsted Hall;

REQUEST BY FAX OR MAIL

    • You may submit the completed form by fax: +63-74-442-3638
    • You may also submit the completed form to our office by mail. Print, fill-out, sign, and mail the form to the following address:

REQUEST BY SCANNED COPY OF COMPLETED / SIGNED FORM

    • You may submit a scanned version (as an email PDF attachment) of the completed and signed Office Transcript Request form. Email to: registrar@brentbaguio.edu.ph

The document/s can be provided only for those whose accounts with Brent International School Baguio are in good standing.

In the absence of the student / alumni, please bring the following:

  • Authorization letter signed by the person requesting;
  • Photocopy of a valid ID of the student / alumni;and
  • Photocopy of a valid ID of the person authorized to pick-up the document /s

The first official transcript is free. For succeeding requests, the corresponding fee/s must be paid upon application at the Cashier or through a bank transaction.

Fees and Charges

    • PHP 200.00 or USD 5.00 for certifications (Attendance, Graduation, Co-curricular activities, etc.)
    • PHP 400.00 or USD 10.00 for Official Transcript of Records
    • PHP 300.00 or USD 7.00 for Unofficial Transcript of Records (for evaluation ,etc.)

Additional Fees and Charges

    • USD 15.00 – bank service charge
    • To be determined – DHL Courier Fee (depending on weight and destination )
